The Problem
Growing a dental service organization means more than adding locations. It means making sure every doctor, no matter where they started, is diagnosing the same way.
As Innovate 32 grew from 10 founding locations in Nashville, Orlando, and Houston to more than 65, new doctors arrived skilled in specialty CE, implants, extractions, grafting, and clear aligner orthodontics, but without the core diagnosis and treatment planning skills.
“Comprehensive diagnosis and treatment planning are what everybody skips.”
— Dr. Dwight Peccora
New doctors out of dental school were promised the exciting, high-dollar procedures, crowns, implants, and full-mouth cases, but without the diagnostic judgment to know when a patient actually needed them. That gap showed up as doctors quietly underdiagnosing patients until small fillings became root canals, or overdiagnosing and recommending more treatment than a patient needed. Without a shared, prescriptive curriculum, that variability began to show up as:
- Inconsistent diagnosis and treatment planning from doctor to doctor
- New doctors chased high-dollar procedure CE without realizing they were missing the fundamentals comprehensive diagnosis and treatment planning
- Supervised neglect, where large fillings turned into root canals, malocclusion undermined new restorations, and undiagnosed periodontal disease led to systemic complications
- Case acceptance suffered when doctors couldn’t explain treatment in a language patients understood
Dr. Peccora wanted an approach that gave every doctor, new graduate or experienced clinician, the same diagnostic foundation, backed by data rather than guesswork.
The Solution: Two Development Tracks Built on Data
Innovate 32 partnered with Spear to build a prescriptive, cohort-based education model rather than leaving clinical development up to chance. The partnership produced two tracks: the I32 LaunchPoint Doctor Development track for doctors in their first five years of practice, designed to teach that dental school is the beginning of clinical practice and not the end, and the I32 Elevate Doctor Development track for doctors ready to grow beyond foundational GP care.
“You can use data to better understand where your doctors might be struggling.”
— Dr. Dwight Peccora
Rather than pulling a course library off the shelf, Spear customized the approach around real performance data. Doctors falling below $4,500 in daily net production and showing low diagnostic benchmarks were routed into the LaunchPoint track for targeted development, so training matched where each doctor actually needed to grow.

I32 LaunchPoint for new graduates
I32 Elevate for experienced doctors
The Result
One cohort of dentists from Tennessee, Houston, and Florida increased total productivity by about 40%, raising their average crown diagnosis rate from 9% to roughly 13%, in line with the national benchmark, simply by learning to identify long-term solutions rather than patching teeth.
“Filling in those gaps is where we’ve really made a massive difference. And that’s where we’re most excited about it.”
The confidence gains reached beyond the numbers, too. Doctors report a new sense of joy in caring for patients now that they feel more confident and capable. “It’s reduced the stress load of clinical care massively,” Dr. Peccora says. Case acceptance rates climbed noticeably across the cohort as doctors learned to communicate diagnoses in language patients could actually act on.
The organization also began seeing stronger consistency in:
Diagnostic accuracy across every market
Case acceptance conversations with patients
Clinician confidence, from anesthesia technique to full-mouth cases
Onboarding for new graduates joining the i32 LaunchPoint track
Recruitment, since candidates recognize Spear’s name as a mark of training quality
